The first mention of it is in the Lovecraft story "The Hound", and Abd Al-Hazred (apparently, that's what it would actually be in Arabic or something) was a name that Lovecraft made up for himself when he was a little kid. The literal translation is something like "Book for the naming of the dead", I believe.
