If it's a small to medium sized image, they make a photocopy of your design using a special stenciling paper. They apply the photocopied image onto the area you want the tattoo applied, so it will look exactly how you want it. They can adjust the size of the image, according to how you want it. You usually make an appointment to see an tattoo artist, so you can have time to talk to him/her about how you want the tattoo done. Plus, it gives you a chance to get to know your artist and how he/she works. Ask to see his/her portfolio; most artists take photos of their work.
