The insane know that they're insane? How could they possibly know that?!
If they had the power of transcendence, they'd not only be able to look upon themselves but also farther out onto the norm of masses and really acknowledge that they may look a wee bit silly in comparison.
Then again, with a gigantic, veinous, pulsing, throbbing super-ego would they really care?
