The hotel level in Shrapnel City in Duke Nukem 3D, where you go to discover the secret level.

Anyway, there's a secret area there where you can see Indinana Jones impaled on a sharp rock. There's also an atomic health thingy there on a pillar. When you jump on it to get it, in true Indiana Jones fashion, the ceiling starts to come down. And it comes down fast. You have barely enough time to make it out again.

Also, the goddamn minigames where you collect the coins in Billy Hatcher. Damnit, those things are nigh-impossible to beat.
