My worst dream ever was once when I dreamed I had to go take a piss, and I was standing over the toilet for like ten minutes and it just wouldn't stop coming, and then I felt something weird and I looked down and a weird, red veiny looking thing was coming out of the hole.
In a similar thread on SomethingAwful someone had a very similar dream so it must not be too uncommon
