Doom 3 - shotgun obvious, plus I modded mine with flashlight. The BFG 9000 also is pretty fun to shoot, and makes you feel invincible. The chainsaw, I just love going through monsters with them not being able to get out of the way.

GTA Vice City & San Andreas - katana is fun to cut heads off. Mini Cannon very fun to see all the vehicles get destroyed, leveling everything in a radius, also it sounds so cool when you fire it. Tear gas is the best, because you can just make people hesitate and then kill them, or just make a run for it. But the best of all is the Desert Eagle it is just has so much stopping power.

Fable - the battle axe with fire augment. It works and looks cool.

For me it's Wirt's Wooden Leg on Diablo 2 (not Wirt's Other Leg on Warcraft 3, which doesn't do anything but add some bonuses). You get to bash thick-scaled monsters with a prosthesis from a dead kid.

Most of the ammo from Oddworld: Stranger's Wrath was pretty neat, but the chippunks stand out. Not many games have ammunition meant to annoy your enemy.

The shotgun with cat silencer from Postal 2

The Sword of Dawn from SOTN was pretty weak, but spawning archers, knights, and mages with it to do your bidding was a cool idea.
Also, the rocket launcher from Halo 2 cause you could lock onto vehicles with it.

Arcanum: 2-handed sword if you're playing an ogre with really high strength. You could chop fuckin' bears in half with it! Literally in half! How cool is that?
Medieval 2: Total War: Elephant Artillery. Okay, I never really got to use them, but come on, cannons mounted on elephants? Damn, that just kicks pure ass. (Okay, basically that's a unit, but gimme a break)
