They have a variety of predesigned patterns (in the thousands as far as I can see from the display rooms of the parlors) that kind of get "stamped" over the skin and the tatto artist just sort of traces along the skin, at least partially, towards the finished work. The only problem with these generic designs is that it usually seems to be assigned to the flunkies because the free-hand work takes longer, is more intricate, and is usually done by the more talented and "in demand" artists. That's kind of a blanket statement, I guess, and I can't confirm that it's 100% accurate. It just seems to be the case from what I've observed.
