I think something like this would be pretty easy to miss, actually. I took a fair bit of graphic design in high school, and it's easy to hide stuff in plain sight because your eye is drawn towards other elements of the layout.

The printer wouldn't necessarily catch it either, since all they do is run off a few thousand copies, of potentially hundreds of different things in a day. Everything is done electronically now, so they probably just got an e-mail or something. Computerized printing presses have pretty much eliminated the need to have a guy standing there checking every copy for flaws, and since it looks like this was an inside page, the guys who box and ship the flyers wouldn't have looked either. :/
