I think the emphasis on the needle in the eye wasn't so much for huge plot importance, as it was the patients freaking the fuck out from being put through all the horrible tests, having the needle in their eye to activate the marker wahtsits, and then wiping their memory, leaving them with a huge amount of paranoia and a very vague recollection of the needle. |
Also, Stalkers weren't a huge problem my first play-through on Normal; they creeped me out when I'd notice one peeking at me, but that was it. On Zealot, they
annihilated me over and over.
